Personal information is collected during conversations with you over the phone, on Zoom or face-to-face, through your use of this website, from your referring Doctor or any other persons/entities that have referred you and when you complete Jodi Rainbow and Wildflowers Therapy forms or paperwork. However, sometimes we may obtain health information about you from a family member when it is not practicable to obtain it from you.
